Medicare CDM/TCA

For persons with a chronic disease or complex health needs your GP can refer you to Physiotherapy as part of a Chronic Disease Management (CDM) plan or Team Care Arrangement (TCA), previously called a Care Plan. Medicare will provide a rebate of $60.35 per session for up to 5 sessions as determined by the GP. For example, for someone with Type 2 diabetes and an arthritic knee your GP could choose to refer you for three sessions of Physiotherapy and two sessions with a Dietician.

Regenerate offers a “gap-free” service with a recent graduate Physiotherapist. If you wish to see a Senior Physiotherapist for a more complex issue we charge our normal private rates and you will receive the $60.35 back from Medicare. The out of pocket expense is approximately $55 for the initial consultation and $45 for any subsequent sessions. If you have a pension or health care card then the gap would be $45/$35. Please note if you have private health insurance you cannot use Medicare and private health for the same appointment.

To book to see a Physiotherapist under Medicare please call 9284 0388 and make sure you mention it is for a Medicare care plan. We will need you to email the referral before your appointment (an emailed photo of the referral is fine) if we have not received it from your GP in advance.
